Bucket Dunk Consent Form 

Congratulations! You have been nominated by your peers to participate in the United Way of Clallam County Dunkin' for Dollars Bucket Dunk! 

Once a final roster of nominees has been determined, the community will be invited to vote for who they want to see dunked. Voting will be open for two weeks. A $5 donation will grant one vote, and donors may submit as many $5 votes as they wish without limitations. The nominees with the highest number of votes [the Dunkees] will be subject to the Bucket Dunk. The Dunkees will sit below a single suspended bucket of water, and community members may donate to throw bean bags at a target to overturn the bucket of water onto them. 

Dunkees will receive a warm beverage of their choice and towels after the dunking. However, the greatest reward of all is knowing the impact you have made in our community. Participation in this event is fully voluntary. 

By providing your signature on this form, you consent to participate in the Bucket Dunk Challenge as a potential Dunkee, with the understanding that you may be subject to receiving a bucket of cold water over your head while United Way staff and members of the community cheer you on.
